Create Survey Item

The Create Survey Item screen is used to construct survey item tests.  These are tests where the answer consists of an option choice.  Option choices may be specified as a continuum or as individual customized options.  Use the Create Survey Item selection on the Build Menu or access the screen directly with the Create Survey Item   button.

 

 

The Create Survey Item screen is used within Survey Type tests to create items for collecting the results from surveys.

 

 

Field

Description

 

 

Test Name

When a test has been selected, the test name is displayed in this field.

 

 

Graphics/Clip

The Graphics/Clip Field is used to display the name of the JPEG or GIF file selected for display.

 

 

Sound File

The Sound File Field is for the display of the name of a selected sound file.  Sound files may be of type ".wav",  ".au", ".mid" or "aif" formats.

 

 

Enter survey question here

Use the Enter survey question here text area to enter the question to be answered.

 

 

Instructions

The instructions for each survey item may be changed.  The default instructions are “Select the option which most clearly indicates your opinion on this issue, from 1 (Strongly Agree)  through to the last option (Strongly Disagree).

 

 

Standard

Two types of survey items may be selected.  Clicking the Standard check box results in surveys with the number of options indicated by the value in the Increments Field.  There is no text associated with these options.

 

 

Increments

The Increments Field is used to enter the number of options to display for a Standard Survey item.

 

 

Custom

Two types of survey items may be selected.  Clicking the Custom check box results in surveys where each option may include associated text.

 

 

Descriptor

The Descriptor text area is used to enter option text for Custom Survey items.  After text has been added in this area, press the Add Button to append the option.

 

 

Press the Add Button to append text in the Descriptor text area to the custom survey option labels.

 

Press the Delete Button to remove the currently selected survey option label.

 

 

Clicking on the field below the Graphics/Clip Field displays a list of sub-directories and graphics files in the current directory.

 

 

Select a sub-directory to search for graphics files in child directories or use the Up Directory Button  to search in parent directories.

 

The Find Button may also be used to locate specific graphics files.

 

Double click on a list choice to select a graphics file to include.

 

 

 

Clicking on the field below the Sound File Field displays a list of sub-directories and sound files in the directory selected.

 

 

Select a sub-directory to search for sound files in child directories or use the Up Directory Button  to search in parent directories.

 

The Find Button may also be used to locate specific sound files.

 

Double click on a list choice to select a sound file to include.

 

 

 

 

The Increments Field has increment and decrement arrow buttons at the right of the field which may be used to increase or decrease the number of Standard survey option radio buttons.

 

 

For Standard Survey items the following optional steps are performed:

 

  1. Enter a question;

 

  1. Select a graphic;

 

  1. Select a sound file;

 

  1. Select the number of options to be displayed by using the Increments arrows;

 

  1. Edit the instruction text area to change the instructions.

 

 

 

Press the View Button to display the Standard Survey item created.

 

 

 

 

The Return Button closes the View Test Item screen and returns to the Create Survey Item screen.

 

 

Whereas the Standard Survey items display options horizontally and do not allow text to be associated with options, the Custom Survey items display options vertically and text may be associated with each option choice.  Click the “Custom” check box to create a custom survey item.  Enter the text of the question in the Enter survey question here text area.

 

 

Use the Descriptor text area to add text for each option and press the Add Button to include each option.

 

In the following example, the text “yes” has been added, followed by the text “no”.  When the text is successfully added, it may be displayed by pressing the arrow in the list field below the Descriptor text area.

 

 

 

 

Press the View Button to display the Custom Survey item created.

 

 

 

The View Test Item screen displays the Custom Survey item created.

 

The Return Button closes the View Test Item screen and returns to the Create Survey Item screen.

 

 

 

 

Clicking on the Goto Button or the field to the right of the Test Name Field displays the survey items which have been defined.

 

 

Double click on the list choice to display the selected survey item.
